Student Registration at LKY School

• Passport
• Copy of passport personal particulars page
• Original documents – certificates, transcripts etc (if applicable)
• Completed ICA medical form, NUS pre-enrolment medical form, Medical report, X-ray report, HIV report (if check-up is done in home country)
• 1 copy of completed ICA medical form and NUS pre-enrolment medical form, Medical report, X-ray report, HIV report (if check-up is done in home country)
• Original Acceptance of Offer of Admission forms (if you have not submitted them earlier)

Medical Examination at Shenton Medical Group Clinic or NUS UHC

• Passport
• ICA medical form
• NUS pre-enrolment medical form
• Signed Shenton Medical registration form from LKY School

Opening of Bank Account at DBS

• Passport
• Student Pass / In-Principle Approval (IPA) Letter
• NUS matriculation card
• Copy of passport personal particulars page
• Book/Settling-in Allowance Cheque (Scholars only) 
• Proof of Residency (Letter obtained from the Student Service Centre)

Student's Pass Application at ICA

• Passport
• Stamped In-Principle Approval (IPA) letter
• Disembarkation and embarkation card
• Duly signed, print-out of Form 16 from SOLAR system
• Medical report, X-ray report, HIV report
• ICA medical form (endorsed by a doctor)

Other Essentials

• Bed Linen – bedsheets, pillows, blankets etc.
• Bath Towel and Toiletries
• At least one set of formal wear
• National Costume (For cultural night, valedictory dinner, etc.)
• Prescriptions, medications, doctors’ letters if you have any medical conditions
